Triangle Area Network is a tax-exempt, non-profit, community service organization created in 1987 as Triangle AIDS Network. TAN was created by a group of concerned citizens in response to the growing health crisis of Acquired Immune Deficiency Syndrome or AIDS.
After more than 27 years the changing landscape of healthcare has led us to expand our healthcare endeavors to include ALL CARE FOR ALL PEOPLE. As national trends are mainstreaming HIV care, TAN has transformed into a full service clinic providing comprehensive and primary care for all individuals in Southeast Texas.
Today, our building, the practice and individuals inside are dedicated to serving everyone in Southeast Texas, including the under-insured and uninsured.
Health centers are community-based and patient-directed organizations that deliver comprehensive, culturally competent, high-quality primary health care services. Health centers also often integrate access to pharmacy, mental health, substance abuse, and oral health services in areas where economic, geographic, or cultural barriers limit access to affordable health care services. Health centers deliver care to the Nation’s most vulnerable individuals and families, including people experiencing homelessness, agricultural workers, residents of public housing, and the Nation’s veterans.
Health centers overcome geographic, cultural, linguistic, and other barriers to care by delivering coordinated and comprehensive primary and preventive services. This care reduces health disparities by emphasizing care management of patients with multiple health care needs and the use of key quality improvement practices, including health information technology.
Most health centers receive Health Center Program federal grant funding to improve the health of underserved and vulnerable populations. Some health centers receive funding to focus on special populations including individuals and families experiencing homelessness, migratory and seasonal agricultural workers, and residents of public housing. The majority of health center operating funds come from Medicaid, Medicare, private insurance, patient fees, and other resources. Some health centers that meet all Health Center Program requirements do not receive Federal award funding. These are called Health Center Program look-alikes.
Health centers leverage a variety of other related programs. Health centers that receive federal grant funding may gain access to medical malpractice coverage under Federal Tort Claims Act (FTCA), and some receive federal loan guarantees for capital improvements.

Make An Appointment
To make an appointment, please call the health center location where you would like to get your care. Our operators are happy to assist you!
For emergencies and unexpected problems, we offer round-the-clock coverage for your health care problems. If you need to reach a health care provider after hours, patients should call one of Our Health Center’s and the appropriate number will be provided.
If you are experiencing a serious or life-threatening emergency, call 911 or go the nearest Emergency Room.
Please plan to arrive 15 – 30 minutes prior to your appointment. This will allow our registration staff time to insure they have all the information needed for your medical records to provide you with quality care and financial assistance as needed.

Once we have reviewed this information, we determine your co-pay on the Sliding Scale. If you do not have insurance, you may also qualify for medication assistance through patient assistance.
TAN accepts most private insurances including Medicare and Medicaid (also Medicaid HMO's). If there is a co-pay or deductible for your plan, payment is expected at the time of service.
If you're uninsured or underinsured (annual deductibles), a Sliding Scale iis provided based upon family size and income which determines the rate of charge. We will work with you to set up a payment plan if it is needed.

It is important that you schedule necessary follow-up appointments, and that you show up for your appointments on time. As an established patient, work-in appointments are accepted as the schedule allows.
