At TAN Healthcare, we believe in removing those barriers. Our dedicated Patient Support Team works directly with you and your healthcare provider to ensure you can reach your healthcare goals. Our Team Members can assist you with:

Medical Case Management: Chronic Care Management: We work closely with you and your healthcare provider to develop and work an effective plan of care specific to your needs.

Patient Navigation Services: We work with you to help address aspects of life that may not relate to your healthcare goals but could impact health and treatment outcomes.

Remote Patient Monitoring:

Evaluation of a medical condition is developing and implementing a plan of care and coordinating medical resources while monitoring the individual’s progress.

Care Management Services:

Helps patients handle aspects of their lives not necessarily related to healthcare needs but which might impact health or treatment outcomes.

TAN Healthcare Patient Support Services can include but are not limited to:

  • Medication Assistance

  • Access to Community Resources

  • Setting Obtainable Healthcare Goals

  • Referral for Substance Use Disorder or Mental Health Services

Transportation
Services

Studies have shown and we too, recognize that the lack of affordable transportation to and from your medical provider is a barrier to accessing quality and affordable healthcare.   TAN Healthcare offers pre-scheduled transportation services to patients for primary, behavioral health or specialty care services in and around the Golden Triangle.  For more information about our Transportation Services, please use our Request an Appointment link and our Care Manager will respond.

Options are based on patient need and medical treatment assigned by your doctor. These include for individuals who are eligible:

The One Year to a Healthier You (OYHY) program is designed to work in collaboration with you, our patient, to create a healthy lifestyle and manage your chronic health condition.

At TAN Healthcare, we understand that sustained lifestyle changes are done gradually, over time.  It takes time to learn how to manage your chronic health condition, secure the necessary medications and medical equipment to do so.  Through the OYHY Program, you will have dedicated care management and support for up to one year after the initial referral from your TAN medical provider.

What is the OYHY Program?

The OYHY program currently focuses on 5 chronic disease conditions:

  • Diabetes
  • Hyperlipidemia
  • Hypertension,
  • Tobacco Cessation
  • Unhealthy Weight Management.

FAQ

1: How can I become a participant?
A referral from your TAN Healthcare provider is required.  For patients eligible for patient assistance, additional documentation may be requested.

2: What is the goal of the Program?
Our goals are YOUR Goals!  Your medical team will work with you to ensure that you are learning about your health through education and motivation, thus resulting in better outcomes. We want to help you increase your participation in an active approach towards better health, increased adherence to medications and preventative care, and reduction in possible disease complications.

3: What if I am not a patient of TAN? Can I still use the program?
You have to be a patient of TAN Healthcare to participate in the OYHY program. A referral from a TAN Healthcare provider is a prerequisite for the program. For more information on how to become a patient of TAN, visit us at www.tanhealthcare.org.

4: How much does it cost?
The program is free.
